REASON FOR LOSS:

F/Lt Reginald Sydney Miller, 87460, was killed when his Typhoon EJ916 'FM-M' crashed on take-off from Warmwell Airfield in Dorset. Squadron O.R.B. states as follows, Scramble “X” Raid. Pilot was taking off across wind at the same time as a Fleet Air Arm Miles Master was taking off into the wind, F/Lt Miller tried to avoid the Master by climbing over it, but stalled and dropped a wing. He crashed on the aerodrome and was seriously injured, dying an hour later. The usual scramble signal – a two star red cartridge – was fired from the dispersal hut, but was not seen by the pilot of the Master. A Court of Inquiry is investigating the accident.
